Operative Note Procedure: Cystoscopy with biopsy of bladder and stent placement Preoperative Diagnosis: Hematuria, suspected bladder lesion Postoperative Diagnosis: Ureteral stone, erythema of bladder wall Indication: This is a 42 year old patient with a past medical history significant for urinary tract infections, who presents due to hematuria and suspected bladder lesion. Procedure Detail The patient was brought to the operating room and placed in the supine position. After confirming the patient's identity and surgical site, the patient was administered appropriate anesthesia. The abdomen and genital area were prepped and draped in the usual sterile fashion. A rigid cystoscope was introduced into the bladder through the urethra under sterile conditions. The bladder was inspected systematically. Findings included erythema of the posterior bladder wall, this area was biopsied. The cystoscope was further advanced into the bladder, and the right ureteral orifice was located. Using a ureteral catheter and guidewire technique, the ureteral orifice was cannulated, and the guidewire was passed into the ureter under fluoroscopic guidance. The ureterscope was passed for evaluation. A large impacted calculi was noted. A 5 French/6 French Double-J stent was then advanced over the guidewire into the renal pelvis, ensuring proper placement. The stent was confirmed to be positioned appropriately with fluoroscopy. The stent was left in place with the proximal coil within the renal pelvis and the distal coil within the bladder. The catheter was removed, and the bladder was again inspected to ensure there was no significant bleeding or other complications.
