Client reports a debilitating onset of fatigue while vacationing in Florida the week before. Client states the heat was intolerable and just wanted to rest in bed most of the time. Neurologic: awake, alert, oriented x 3; pupils equal and reactive to light and accommodation; corneal reflex present bilaterally; extraocular movements intact; hand grasps equally present but weak; bilateral weakness of upper thighs present Cardiovascular: apical 68 beats/min and regular; no jugular vein distention; pulse present and equal bilaterally; no peripheral edema; capillary refill 2 seconds Respiratory: breath sounds present and clear over all lung fields; no cough; mucous membrane moist; nailbeds pink Gastrointestinal: bowel sounds present x 4 quadrants; abdomen soft, nondistended; nontender upon palpation Musculoskeletal: slight tremor noted on left hand; muscle spasm of left calf.
