Patient scenario:
Mr. Vesser is a 79-yr-old, single, white male admitted to the nursing facility for convalescence following a tracheotomy for obstructive sleep apnea. Before the surgery, he lived alone. He describes himself as limited in activities such as driving, traveling, and cooking because of his respiratory distress. He reports daytime fatigue associated with grooming, dressing, feeding, and toileting. He reports sleeping poorly and is awake several times during the night. He reports general fatigue all day long. As a result, he takes a daytime nap. Past medical history includes: hypertension, obesity, chronic obstructive pulmonary disease, severe peripheral vascular disease with a stage II venous stasis ulcer present on the lower leg, and recent tracheotomy for obstructive sleep apnea. While at the nursing facility, Mr. V. tells you that he plans to discharge himself home in one to two weeks. He is observed to need assistance with ambulating. He refuses to go the dining room and requests a refrigerator in his room. He eats all of his meals in his room and rarely socializes with any resident or staff member. He spends his time playing solitaire in his room and watching television. He is a retired salesperson and worked in business 40 plus years. He says he does not want to try to make friends as he will be going home soon and will not be in contact with these people after that time.
