The patient is a 54-year-old Caucasian male came in for this appointment made by his sister. His symptoms are visual and auditory hallucinations. The patient reported seeing the shadows of people watching him through his window. He stated he does not sleep well because the voices are loud and keep him up for days. He reported he tries to watch TV but the people are watching him through the TV screen. The patient stated that the people come in and poison his food. He refuses to take his medications, he stated they are all poison. He stated the government is tapping his phone. He reported hearing heavy metal music and seeing birds flying in the room. The patient stated he has no legal issues but has been warned if he did not stop calling 911 about the people outside his window he will be arrested. The patient reported he smokes three packs of cigarettes a day and drinks 12 packs of alcohol a week. The patient denies any blackout or seizures from drinking alcohol. The patient reported physical abuse by his dad growing up. He is no currently on any medications for his mental health, he is non-compliant with them because he said they are all poison but he is on metformin for diabetes. He has had Haldol, Thorazine, risperidone, and Seroquel in the past.

QUESTION

Provide a treatment and management plan for this patient including alternative therapies, psychotherapy, pharmacologic and non-pharmacologic treatments and follow-up parameters with rationale. Incorporate one health promotion activity and one patient education strategy.
