Discharge Summary
Health record of a three day old baby boy. The patient is a 3-day-old male infant. He was born at home after approximately 38 weeks' gestation and brought to the emergency room shortly after thereafter with difficulty breathing. The baby's respirations were very quick and shallow. This was his mother's fourth child, all of whom have been born at home. This was the first child born with complications. The newborn was resuscitated and placed in the NICU under continuous oxygen therapy. Admission history and physical examination were otherwise unremarkable. Chest x-ray showed wet lungs. Repeat chest x-ray 24 hours later showed the lungs had cleared. PKU specimen was taken and sent to the state laboratory, as required. By the second day of hospitalization the patient was able to be weaned off the oxygen and discharged from the NICU to a bassinet. The mother requested that the baby be circumcised. This was accomplished without incident. The infant is discharged to be seen in the clinic at one month for a well-baby visit.
