Give a sample rehabilitation protocol for a lower extremity injury of any choosing. When creating your protocol consider using a table/chart to organize some of the information and keep it precise/to the point-see below. Incl

Protocol Set-Up:

Title, your name and date at the top of the document. Introduction paragraphs to cover #1-#4 below. A chart(s) to organize #6-8 below. A paragraph to explain the return to play criteria/component. A conclusive sentence (s) to wrap up. List all references at the bottom of the document.  use in-text citations 

1. List your specific injury
2. Give a definition of the injury and a short description of what types of restrictions a patient with this injury should expect.
3. Give specific information about risk factors for this injury.
4. List common interventions for the injury.
5. Remember the focus that we have discussed all trimester regarding the principles of rehabilitation. Will your protocol need to incorporate treating pain, swelling, range of motion, strength, proprioception, plyometrics, function? That depends on the injury and the symptoms/findings. Make sure your choices in #7 below are explained for their purpose (the reason each is in your protocol)
6. Separate your rehabilitation protocol into the various phases (phase 1-3 plus a return to play section. Include the relative timeline for how long each phase should take. (Using a range for the timeline is recommended). These phases should be based on the stages of tissue healing that we discussed: inflammatory phase, proliferation. and remodeling phases.
7. Under each phase of the rehabilitation, list the exercises, stretches, joint mobilizations, techniques etc., that you would recommend.
8. Be specific about recommended reps, sets, time, resistance.
9. Give criteria necessary for the athlete to return-to-play.
