Question: MaineGeneral Health organization located in Augusta, ME, had high and rising relapse rates for congestive heart failure (CHF). In 2020 MaineGeneral Health initiated a remote patient monitoring project to track congestive heart failure patients after they were discharged from the hospital. The main purpose of this trial was to keep monitoring patients without them coming for in-person appointments, as well as providing information and resources concerning their health condition. At first patients were monitored at home by a specialized nurse practitioner but when the COVID-19 hit they focused on finding a user-friendly option to implement virtual visits. These appointments were conducted via Zoom. Patients recorded their data for remote monitoring, and it was immediately sent to the centralized care team. The whole project was so successful that they managed to accomplish 0% readmission for congestive heart failure patients
