PSPINV003 Finalise an investigation - Tafe Queensland
You are requied to complete a Final Invesrtgation Report.
After several months of investigating where you have visited the scene / location, recorded a number of victim, witness statements, collected, handled, managed, stored all relevant information and exhibits in accordance with legislation and even formally interviewed persons of interest in your investigation. You believe you have now completed your investigation and are ready to write your Final Investigation Report.
You are expected to make a number of recommendations in your report as to the next course of action. These recommendations include the formal laying of any charges against responsible persons by the appropriate agency (DPP etc). You can also make recommendations based on the information outlined in the investigation to prevent the incident reoccurring.
As a GIST investigator you can also recommend what action may be taken against the POI in the absence of prima facie evidence of a serious offence but with sufficient evidence to conduct organisational administrative action or recommend lesser charges; however, all recommendations need to be supported by your investigation evidence.

• You are to use your and student number as your identification details
• The reference number is the applicable number for your allocated scenario
• You are a Senior Investigator
• You belong to the Government Investigative Service Team (G.I.S.T).
Investigation Report Format to be used
1. Annex:
Include the key points from the following sections of the report:
• Purpose
• Findings
• Summary
• Recommendations.
2. Purpose:
Explain why you have written the report; explain briefly what has happened, and what obligations you have to investigate. Are these obligations company policy, or are they legislated?. Include any relevant legislation relating to the investigation.
3. Methodology:
List all the sources of information that you are using in the report. You should only list sources in here that you are using as part of your report and investigation. You should also list the title of the documents - you should not explain what your findings were, however.
4. Findings:
List the facts of your investigation including a chronological explanation of actions taken.
• Outline the details of the incident
• List the various sources of information
• Witness Statements
• Explain what the witnesses saw
• Explain any evidence collected.
5. Summary:
List important conclusions that can be drawn from the findings. Include a summary of future outcomes and whether any further reports are outstanding
6. Recommendations:
Detailed recommendations based on the evidence and outcomes of your investigation.You should make recommendations which will prevent reoccurrence. You may also make recommendations for referral of the subject/suspectto another agency if previous criminal/discipline activity is identified making the matter serious and/or complex requiring external specialist support.
7. Annexures:
In your annexures, you may include copies of documents which you used in your investigation. Examples include, witness statements, transcripts of interviews, photos, copies of documentary evidence. As well as any annexures you choose to include, you must include the following:
• A Witness List
• An Exhibit List.
8. References:
Any reference material used during your investigation including legislation, policies and guidelines.
