Case Study
Belinda Otero, agitated and screaming, was brought to the emergency room of a local hospital by her family, complaining of severe left-sided facial pain. The previous week, after she heard that her former husband had remarried in the Dominican Republic. She had become increasingly agitated, restless, and developed insomnia. She moved into her mother's home with her two children, and that day developed the facial pain.
By the time she was seen by the attending psychiatrist, Ms. Otero was ultimately mute and mumbling unintelligibly in Spanish and English. She was given small doses of an antipsychotic medication with no response. Because of her behavior, she was admitted to the psychiatric unit for further evaluation.
On the unit, Ms. Otero was agitated and had outbursts of bizarre behavior. She appeared sad, restless, frightened, anxious, and disheveled. She received anxiolytic medications, and there was some decrease in her agitation and restlessness, but she continued intermittently to display a bizarre behavior.
On the following day, a family meeting was held with Ms. Otero, her mother, daughter, and son. It now became clear that when Ms. Otero heard that her ex-husband had remarried, she became terrified that he would no longer support her and their children. A meeting was scheduled the next day to discuss these issues at which he indicated his intention to continue to pay child support. After this meeting, there was a pronounced change in Ms. Otero. Her symptoms resolved, and her intermittent episodes of agitation stopped. Her medication was tapered and discontinued without recurrence of symptoms. Having returned to her previous level of function, she was discharged after one week of hospitalization.
