Task 1:

This task (in part) requires the construction of an AcciMap diagram related to a specific accident. You will then be asked to analyse and discuss aspects of the accident in relation to a Safety Management System (SMS) and the aviation industry in general.

Except where the use of additional external references are specifically requested, use the supplied National Transportation Safety Board (NTSB) accident report regarding Asiana Airlines (NTSB, 2014) as the primary reference document for this task.

Question 1A:

Using only information from the NTSB (2014) accident report, construct an AcciMap diagram using the guidance provided by the document Guidelines for AcciMap Analysis (Branford, Naikar, & Hopkins, 2009).

Ensure your diagram provides sufficient interacting factors, including those at higher levels and not just those focussed on the immediate cause of the accident, to provide a clear understanding of how and why this accident happened.

Use the sample AcciMap shown at Figure 2 of Branford, Naikar, & Hopkins (2009), as an example of a completed AcciMap diagram. To assist in achieving this, and to avoid the construction of a massive and complex document that would be difficult to include in your submission, take note of the guidance listed by Branford, Naikar, & Hopkins (2009, p. 203) recommending the inclusion of only two types of causes.

If you are unsure about the computer skills needed to produce the AcciMap diagram, consult the Information Technology specialists at the library or your fellow students (via the Discussion Board?) about techniques to produce the AcciMap diagram as an attachment to your Word document submission. The AcciMap diagram attachment will not be included in the word count for this assessment. If you are unable to complete the AcciMap diagram as part of the Word document, it is acceptable to submit a scanned document of the AcciMap diagram you produce. Methods for submission of a scanned separate document will be advised on request. In all cases, ensure your AcciMap is legible as I will be unable to contact you to seek clarification.

Question 1B

Provide an analysis and discussion of your AcciMap diagram. Your discussion should demonstrate your understanding of how AcciMaps assists in establishing how factors in all parts of a sociotechnical system contribute to an organisational accident (Branford, Naikar, & Hopkins, 2009, p. 193). Use any references you desire to aid your explanation.

Caution: do not simply restate theory, or paraphrase sentences and statements from the accident report (NTSB, 2014) and Branford, Naikar, & Hopkins (2009), or any of your other sources, to analyse and discuss your AcciMap diagram.

Question 2:

The International Civil Aviation Organisation (ICAO) (2013) states the framework of a Safety Management System (SMS) contains four components and twelve elements.

Using each of the four SMS components as headings, and using only the supplied accident report (NTSB, 2014) as a source, identify one aspect of the accident for each of the four components of a SMS, that you feel could relate specifically to that SMS component.

Provide an analysis and discussion of each of the four points you choose, to demonstrate your understanding of its relationship with the specific SMS component. Use any references you desire to aid your explanation.

Question 3:
The Executive Summary of the accident report (NTSB, 2014, pp. xi-xv) provides a list of safety issues discussed in the report. One of the issues relates to the need for: More manual flight for Asiana pilots.

Analyse and discuss this safety issue from two perspectives:

1. Its relevance and application to the aviation industry in general, and

2. What components or elements of the SMS at Asiana you would ensure are in place, to address this safety issue?

Examples are used to display a good understanding.
