Question

A 32-years old man is brought to the emergency department due to the progressive confusion and lethargy over the past several hours. The patient has not significant medical history but he has been depressed since a recent breakup with his girlfriend. He works at an automotive repair shop, and one of his coworkers reported seeing the patient consume antifreeze prior to symptom onset. Blood pressure is 110/66 mm of mercury, pulse is 110/min, and respirations are 24/min. The patient is somnolent and responds only to painful stimuli. Pupils are equal and reactive bilaterally. Fundoscopy is normal. Lung fields are clear to auscultation and heart sounds are normal. Bilateral costovertebral angles are tender to percussion. Bladder catheterization yields a small amount of a bloody urine.
