However, since his wife died 4 years ago, Phil has struggled to 'see the point of all this fuss', doesn't take his medication as directed and has put on weight. Mr Gibson is being seen every 3 months by the diabetes nurse specialist, as she is concerned about how he is managing his diabetes. At the clinic, he has his HbA1c checked; the results indicate a lack of adequate glycaemic control.

Phil admits to having difficulty managing his blood sugars and his medication. He confesses he discontinued the acarbose some time ago because of an upset stomach and bloating. It is decided to start him on a small dose of insulin.
