Question

Patient PEEWEE. is an 8-year-old male referred by his pediatrician for consultation regarding his mood. The patient endorses feeling worried most of the time and having bad dreams most nights. He describes that in the dreams he is lost and cannot find his little brother and his mom. He denies ever experiencing being lost in real life. The patient reported he is afraid of the dark, so he sleeps with the night light on and the door open. Pt reported he worries about his mother and little brother when he is at school, wonders what they are doing at home and if they are okay. He endorses him and worries if his mom will pick him up after school. The patient reported that no one likes him at school, and they call him names like "smelly". The patient reported that his teacher is always telling him to sit down, focus, and stop looking out the window. He reported that he sits at a desk beside the teacher' but he is not bothered because he is away from sitting next to one of his classmates whom they have issues. The patient reported that he has nocturnal enuresis. The patient's mother reported some concerns about his poor appetite as a result has lost three pounds in three weeks. She added that she is concerned about his excessive worries all the time about silly things and how the patient feels she loves his little brother more than him. The patient's father was deployed overseas and was killed when patient was 5 years old but his mother told the patient his dad went on a vacation, the patient worries and cannot understand why he never returned
