After a discussion with Pamela, we have placed her on a mental health plan that includes counselling to continue with you for a total of 12 weeks. At this point, due to Pamela's issues with medication, we have decided not to place her on any medication for her possible depression. We discussed the importance of a healthy diet, exercise and good sleep as alternatives to medication. Once her 12 weeks is complete, please refer her to me for an updated assessment on her mental health plan.
