Management plan for diagnosis lumbar strain for patient a 54-year-old male computer programmer with a PMH of HTN who reports one week of left lower 6/10, achy, constant back pain that radiates to the left buttock with intermittent tingling. He regularly exercises and reports lifting heavy boxes and furniture last week. Activity worsens his symptoms and acetaminophen does not provide relief; however, he continues to exercise. Denies significant trauma, history of cancer, fever, weight loss, or bowel/bladder ncontinence. Physical exam reveals stable vitals with localized tenderness to the left lower back and gluteal region with muscle spasm and mildly decreased ROM of the spine. Bilateral lower extremity strength is 5/5, ROM intact, and reflexes 2+. Straight leg raise, FADDIR, and Frieberg tests are negative but reproduce localized pain.
