Bobby is a 57-year-old Black male who is a long-term psychiatric clinic patient with schizophrenia. His primary support person, his sister, recently passed away, and he is not able to cope independently with medication management and day-to-day life skills. Bobby has been referred to the Program of Assertive Community Treatment (PACT), which involves an interdisciplinary team of healthcare professionals providing in-home and community services to individuals in need.

Bobby's health services are typically supported through Medicaid, but he needs to reinstall his insurance. Bobby cannot find the paperwork he needs to submit for recertification because his sister "always took care of things like that." The house is unkempt, with paperwork lying in various places and no clear path for walking. If Bobby does not get recertified in time, he won't have access to PACT services and necessary psychiatric medications. Bobby does not drive and is homebound aside from regular trips to the grocery store with a neighbor. PACT professionals see him three times a week but are unable to fully support him with activities of daily living.
