Problem

AM is a 32-year-old male with IV drug use and presumed sepsis. The physician orders gentamicin therapy with the pharmacy to dose. He is 6; tall and weighs 175 lbs. His current SCr is 1.0. Use extended interval dosing (Hartford Nomogram) to determine empiric starting dose. Then check what ClinCalc and Global RPh would recommend. The initial dose is given at 6 PM.
