Mr. Jackson is a 55 year-old male diagnosed with high blood pressure two years ago. He works at Home Depot and lives a moderately active lifestyle. His blood pressure have been consistently high, and his doctor prescribed him a diuretic: hydrochlorothiazide, to manage his high blood pressure.

Despite being aware of the importance of taking his medication regularly, Mr. Jackson struggles with medication adherence. He would forget to take his medication and sometimes skips doses. He stated that the diuretic makes him use the bathroom frequently while at work. This erratic medication behavior has led to inconsistent blood pressure control and puts him at risk for complications related to hypertension.
