Question

A 27-yearold female presents to the Emergency Department (ED) wo days postpartum. She is Complaining of severe abdominal pain and reports some vaginal bleeding She reports an uncomplicated pregnancy and a normal vaginal delivery. She and her husband both appear very anxious.

1. List two initial nursing assessments that need to be performed for the patient.

Upon further assessment patient is pale and weak. Pulses are 1+ bilaterally in radial and pedal arteries. The Enrolled Nurse (EN) obtained vital sings and assisted patient to the bathroom and noted a sanitary pad saturated with bright red blood. Her urine was dark yellow and clear Vital signs: Blood Pressure 116/72 mmHg. Heart rate 92 bpm and regular, Respiration Rate 22  bpm, Temperature 36.6'C. Sa02 96% room air

: Physical examination

-Pale, cold, and clamy Weak drowsy but orientated

You take another set of vital signs and note the following.

Vital signs: blood Pressure 108/68 bpm and regular. Respiration rate 28bpm Temperature 36-4'C. sa02. 94% on room air.

2. From the patient's physical examination, there were abnormal findings including her blood pressure, heart rate, respiration rate and oxygen saturations Choose one (1) of these abnormal findings in relation to patient's presentation, Discuss and justify with relevant pathophysiology.
