Task Overview
In Part B of clinical assessment you will be producing two pieces of evidence from your clinical practice.
Both pieces of evidence are under LO1 as part of professional development of your practice. The case review section should reflect the care you provided to a patient you cared for in clinical practice. Get the best assignment help from top tutors.

2. One case review of a patient from your clinical practice using the NIRR framework (LO1)

one reflection of a situation involving your care of your patient using the Gibbs Reflective Cycle.
and
one case review of a patient you cared for in your clinical placement using the NIRR framework.
1. Reflection of a situation involving care of your patient.
Choose a situation from your practice during your clinical placement.
This can relate to any aspect of care but it must be a patient you assessed and planned care for.
Identify a particular aspect of the care you provided that either went well or did not go so well.
The reflection will focus on what you have learned from the situation and how this can help you develop your skills and knowledge.
The reflection should be structured with headings using the Gibb's Reflective Cycle (refer to Reflective Writing Resources module).
The analysis section should be supported by evidence from relevant literature (use the library search functions and the course resources/textbooks).
Use APA 7th referencing style.
You may also look to the NCNZ nursing competencies, the health consumers code of rights and Te Whatu Ora policies and practices to support your reflection and analysis of the situation.
The upper word limit is 1000 words. Aim for between 750 - 1000 words. Your writing should be as concise as possible.
Use the word document and template provided with the Gibb's Reflective Cycle headings. Upload the one document that has both pieces of evidence.
2. Case Review of a patient you cared for during your clinical placement.
Choose one patient you focused on and cared for.
Use the Notice/Interpret/Respond/Reflect (NIRR) framework headings to organise the review.
There should be no identifying details that could breach confidentiality. Anonymise all names and places.
Include demographic details - e.g., gender, age, ethnicity and a brief summary of the patient's social situation.
Provide reason for admission, main presenting condition, how many days the patient has been in hospital and a brief overview of the relevant past medical history.
If using references from a textbook or journal articles please provide the correct in text citations and references using APA 7th style.
Include a list of the patient's current medications and a brief explanation of reason for administration.
The review should cover a brief summary of the presenting problem/s that led to admission to your ward.
Identify the main patient-centred problems for your patient. For example, shortness of breath, impaired mobility, pain, anxiety but the problems should be derived from your patient's assessments.
Use the NIRR framework to present how you managed this patient's care.
