Problem
Using the charge master index attached as a basis consider what would happen if the elements on the CDM were outdated and what and when each need to be reviewed to ensure accuracy and compliance.
Using the P&P template Download P&P templateprovided, make a CDM maintenance policy and procedure. At a minimum detail the:
I. Purpose - basic explanation of the policy -
II. Scope - defines boundaries of the P&P and identifies who is affected by the policy and procedure (stakeholders)?
III. Describe the policy/policy statement - The reason or rationale for the policy and what the policy is intended to accomplish. Include how the CDM relates to the revenue cycle .
IV. Definitions: define each element contained in a typical CDM (revenue codes, CPT codes, etc.) and acronyms, abbreviations, or terms referred to in the procedure section.
V. Procedure - specific steps necessary to comply with the policy (this is the 'how'). List the high-level steps that are needed to make sure the CDM is accurate, up to date, and compliant on an ongoing basis:
1) Identify necessary tools or sources of update information and compliance with (i.e., CPT/HCPCS manual, CMS and other pertinent sources).
2) Identify responsible person(s) or department(s) (CDM committee/team). Specify who is on the CDM committee.
3) Identify those specific elements of review that must be included in an audit (i.e. deleted codes, additions, changes, modifiers, fee schedule updates, etc.).
4) Include timelines (frequency of maintenance; quarterly, semiannual, annual, etc.). This should not be arbitrary. Consider when codes are updated annually and periodically throughout the year.
Organize the elements above into a usable procedure. The policy is the 'why' and the procedure is the 'how'. This should be a clearly outlined process with identified steps, departments, and roles. If you are unfamiliar with a policy and procedure, you may need to do some light research.
