Support Community Participation And Social Inclusion
Section 1: Preparation
In this section you will need to prepare for your meetings with each person. If you have not already, read the information about each person in the CHCDIS012 Simulation Pack. Complete the table below, providing as much detail as you can.

Answer the following questions about Murray Hess.

Section 2: Assessor observations
The following must be directly observed by the assessor. You must support at least two people with disability to identify skills and interests and find matching options within the broader community, including:
• selecting options that meet needs and preferences outlined in their individualised plan and supporting them to access
• identifying barriers and implementing strategies to overcome them in collaboration with the person.
Client 1: Murray Hess
Did the student:
Apply person-centred principles of practice?
Apply a strengths-based approach to practice?
Consult with Murray to identify his interests, needs, abilities and preferences regarding social networking?
Identify, access and provide Murray with information about community participation resources, programs, agencies, transport services, aids and equipment?
Identify and accommodate Murray's cultural and religious needs (as applicable)?
Support Murray's use of assistive technologies in meeting his individual needs?
Discuss with Murray any gaps in his needs for assistive technology?
Support Murray in identifying ways in which he can establish connections with others through shared interests?

Discuss with Murray to identify areas where advocacy is required to help him engage in his preferred choice/s of participation?

Section 3: Assessor observations
In this section of your Portfolio, your assessor will observe your performance during each role play.
The following must be directly observed by the assessor. You must support at least two people with disability to identify skills and interests and find matching options within the broader community, including identifying, addressing and monitoring barriers to community participation and social inclusion.
Did the student:
Seek feedback from Murray to ensure support is meeting his current needs and preferences?
Review Murray's strategies for community participation and social inclusion to ensure outcomes remain positive?
Monitor Murray's levels of engagement in community participation and social inclusion?
Recognise any new barriers to community participation and social inclusion?
Support Murray in identifying solutions to barriers and helping him implement strategies accordingly?
Discuss the success of strategies applied to deal with barriers originally identified?
Section 4: Reflective journals
Reflect on your work with Murray and Omar. Complete a copy of the Reflective journal for each.
Explain how you applied the principles of strengths-based practice and person-centred practice with Murray.
Explain how you supported this person to identify their interests, needs, abilities and preferences in regards to community participation and social inclusion.
Explain why you believe the options you identified at Section 1 or your Portfolio and during the role play with Murray were suitable.
During your work with Murray, was there any time when you felt you needed support from your supervisor?
If yes, provide details. If no, consider a situation with Murray in which you might have needed your supervisor's assistance.
During the initial role play Lourdes was encouraging and supportive of Murray. Assume she had attended the second meeting but was very negative about what Murray was doing - even though Murray was finding enjoyment in most of his experiences.
What strategies could you have applied in this situation to address her attitude?
Explain how you applied the principles of strengths-based practice and person-centred practice with Omar.
Explain how you supported Omar to identify their interests, needs, abilities and preferences in regards to community participation and social inclusion.
Explain why you believe the options you identified at Section 1 or your Portfolio and during the role play with Omar were suitable.
During the follow-up meeting with Omar, you discussed ways in which he needed support at work.
Explain the steps you would take to work with his employer and ensure Omar's ability to fully participate and be included in the workplace.
During your work with Omar, was there any time when you felt you needed support from your supervisor?
