Question - Improving Electronic Medical Record Documentation Case Study

Identify a process or an opportunity for improvement at your place of employment. If you are not currently employed in health care, this process will still be relevant. If you are not currently employed, think back to your last health care job and identify an issue that would have benefitted from an improvement plan. Synthesize a process improvement plan using the following modified FOCUS/PSDA format to work through to a solution for the item that needs improvement.

Step 1: Identify an opportunity for improvement. This could be in a process or it could be an individual issue that seems to cause problems for the staff or the institution. You may need to discuss this with some of the leaders/managers at the institution or even discuss this with your peers to get some ideas.

Step 2: Review the current process in regard to the problem/issue/process to gain background information. Review any regulatory issues that might be impacted. Is there any data that would support a change or any data that would hinder you from putting an improvement plan into practice? Does the current process have evidence that it is best practice?

Step 3: Propose a team of people that would be needed to work on the problem/issue/process.

Step 4: Identify all of the resources that you feel would be needed to implement an improvement plan. Think about time, materials, staff, external needs, cost, etc., that would be needed.

Step 5: Outline your solution for the improvement process. What evidence do you have that this solution meets evidence-based practice standards?

Step 6: Put together the plan for improvement with all of the above elements covered and the indication of how the change would be implemented (the change strategy you would utilize).

Step 7: Your report should be 4-6 pages in length. Be sure the report is in MS Word, using APA format with a minimum of three scholarly references and 2 citations. The length is not inclusive of the title and reference pages.

Outline a one-page synopsis of your final project and place it in the discussion forum for the project.
