Action Plan: Losing Weight to Promote Health
1. Introduction to the topic or behavior you want to change and provide specific information from scholarly literature to support the need for this change.
2. Identifies one goal for desired behavior change and supports goal by scholarly literature; specific, measurable, realistic, and individualized actions to achieve the goal (specific actions should include answers to questions such as what activity, why, where, how much, when) are included.
3. Identifies confidence to achieve the goal using a numeric scale. The scale to be used is 1 being not confident and 10 being very confident.
4. Identify some of the barriers or challenges you anticipated to prevent you from achieving the goal and those that actually occurred, potential solutions used to overcome the barriers.
5. Identify what specific activities you will engage in to reach the goal and how those activities will be monitored; Identify how activities to reach the goal will be documented.
6. Identify if the motivation to change was extrinsic or intrinsic and why you believe this is so. Identify the importance of implementing the action plan and why you selected this level of importance.
7. Identify whether or not you met the goal. Discuss the actions that helped to meet the goal and which ones did not. Discuss how the goal or actions might change if you were to continue working on this area and the next step in continuing the plan.
8. Provide some introspection regarding what you were able to glean from this action plan and the work you put into the activities for this action plan, including motivation, self-management, adherence, self-awareness, use of action plans with clients, or other areas.
