Scenario - Karen

You are an Enrolled nurse working for a community home care service, each week you go to Karen's house to help her get ready for her weekly hydrotherapy treatment that helps to improve her strength and coordination. You generally get Karen ready, take her to the hydrotherapy and bring her home. Karen uses a wheelchair as she has cerebral palsy.

Karen is usually enthusiastic and happy to chat with you while getting ready for these sessions, however today Karen seems down and only mumbles a greeting when you say good morning.

You ask Karen if everything is aright, she says, "I don't want to go to therapy, why does everyone bother with me, I'm not worth spending time with. I'm better off dead"
