You are aware, the duties of your position require that you handle day-to-day work challenges effectively and achieves results by adjusting to multiple demands, ambiguity, and rapid change. Therefore, for me to consider your request for reasonable accommodation, I will need medical documentation that establishes the existence of your medical condition/disability.

The medical information should:

1. Provide an explanation of the symptoms you are experiencing (prognosis, duration, nature and severity of the condition);

2. Provide an explanation of your limitations.

3. Explain how your limitations impact your daily life activities.

4. Explain how your limitations affect your job performance.

5. Explain what job tasks are problematic for you, because of your limitations; and

6. Specify the reasonable accommodation requested, as well as how the reasonable accommodation will assist you in performing the essential functions of your job.

The medical condition is diabetes and I need to make a case for reasonable accommodations to work from home.
