Problem

The obstetrics department at your facility were slow to adopt the EHR platform. Because they do so much at bedside with the baby, like obtaining the footprints, they also prefer to use carbon copy forms to give the mother copies of any vaccinations given to her or the baby. In comparison, obstetric patients that have a C-section are followed up by surgical OB and primarily have all documentation completed in the EHR. Babies that are born via SVD tend of have around 40-60% of their documentation completed on paper. One of the OB physicians also uses a printed form to complete delivery information. Currently, the completed medical record on C-section patients are completed within two days of discharge. Vaginal birth paper records taking 4-7 days to arrive in the HIM department for scanning. The current A/R days is 3 days of discharge. Evaluate the following: Why is there be a discrepancy between C-section verses vaginal birth records in terms of availability for coding?
