Question

A 65 year old male is referred to the Emergency Department by his GP with a 4 week history of a cough initially productive of yellow sputum that has more recently become blood stained. In addition, he has had intermittent fevers and night sweats over the last few weeks associated with a 7 kg weight loss.  His GP initially treated him with a 7 day course of amoxicillin followed by a 7 day course of clarithromycin with no improvement.

He is a non-smoker and is originally from Lithuania but has been living in Ireland for the last 10 years.

He was last in Lithuania over the summer for a 2 week period. He shares a house with 2 other men who are also from Lithuania.

On examination:

• He appears underweight.

• His respiratory rate is 14 breaths per minute, his heart rate is 70 bpm and his blood pressure is 110/75 mmHg.

• On auscultation of the chest there are reduced breath sounds in the right upper zone otherwise examination is unremarkable.
