First, identify an issue which is hindering the employee's performance. You may choose any issue which is relevant in your workplace. The performance issue could involve using the Internet on company time. After you identify the issue write a 1-2 paragraph problem statement which identifies the employee's performance issue and includes any necessary background information or data about the situation. Finally, develop 5-8 probing questions that you will ask during the interview. These questions should be "what" and "why" questions to help you determine the cause of the employee's performance issue. Ask open-ended questions which will allow the employee to elaborate and will allow you to ask further questions
