Baby J was born 2 days ago at a freestanding birth center run by certified nurse midwives. Patients there receive a lot of patient education during their prenatal visits, including how to monitor themselves and their newborns once they are home. Client and baby were discharged six hours after birth in stable condition. As instructed, the client monitors and records vital signs, amount of bleeding, location and firmness of the fundus, baby's vital signs, behavior, feedings, voiding and stooling. The protocol at the birth center is for a phone check in on day 1 after the birth and a home visit by a nurse on day 2. During the home visit the nurse notes that the baby is active and alert; latching on and breastfeeding well; vital signs are T- 97.9, HR 148, RR 36 when quiet without grunting or retractions; and skin and sclera are slightly jaundice. The grandmother is there and is quite verbal that her daughter should have given birth at a hospital with a doctor, should not be breastfeeding and that the baby is yellow because they are not getting formula. The client is becoming tearful and asked the nurse if formula would be better for the infant.
