Alan is a 37-year-old banker, who was divorced from his wife two years ago. Alan reported that his wife left him after he became disengaged from the marriage. He recalled that he and his wife were college sweethearts and were previously very active in their community. Then, approximately five years ago, Alan said, I just ran out of steam. He has since been constantly irritable, started sleeping excessively, gained about 45 pounds, and lost interest in being social and engaging in pleasurable activities.
Alan smokes marijuana approximately two to four times per day and drinks vodka nightly to relax and take my mind off of things. He recently was arrested for possession of marijuana and driving under the influence, put on probation for one year, and was referred to counselling by the court. Alan admits to being mildly depressed, but insists, It's nothing I can't handle. He does not wish to discontinue his marijuana or alcohol use but has thoughts about stopping due to monthly drug screens, which will soon be required by his probation officer.
1.) Formulate a DSM 5 diagnosis based on the case content. Include ICD-10 numeric coding, official wording for the disorder, and any specifiers that may be applicable.
