The first step in considering treatment with an antibiotic is to determine whether or not there is an infection. What aspects of history indicate their is increased potential for urinary tract infection in this patient that would raise your suspicion for UTI? Dysuria, fever, urgency, frequency, recent intercourse are all signs of or risk factors for the development of UTI. The urinary tract, from the kidneys to the urethral meatus, is normally sterile and resistant to bacterial colonization despite frequent contamination of the distal urethra with colonic bacteria. The major defense against UTI is complete emptying of the bladder during urination. Other mechanisms that maintain the tract's sterility include urine acidity, the vesicoureteral valve, and various immunologic and mucosal barriers.
