Uterine Contraction lasts for 25 to 30 seconds with an interval of 2-3minutes. Vital Signs: T: 37.5'C, Pulse 93, RR: 22, Bp: 120/90 Pain Scale: 9/10. FHT: 150 bpm.
The Doctor made the following orders:
Transfer patient to the delivery room

Monitor VS q 15 and record pls
Monitor I and O q1
Attach patient to fetal heart monitor
Administer fleet enema
Provide perineal care
Empty client's bladder using straight catheter
