Ms. Knowles has been fairly compliant with preventive services since the completion of the on boarding process. Due to an incident that occurred a couple of months ago Ms. Knowles and her family were placed in a shelter in Queens. After several weeks Ms. Knowles was discharged from the shelter as she had already found an apartment prior to the incident which was in lease agreement. Ms. Knowles moved to 2764 Webb Avenue Apartment 1B. Since the arrival to the new home preventive has been unable to meet with the client due Ms. Knowles being unable to meet. Recently Alicia Ms. Knowles's daughter gave birth to a baby boy. Kimora who is Ms. Knowle's granddaughter is engaged in Mental Health Service with Fordham, to which she is compliant. Amiyah is enrolled with Mental Health Services through New York Psychotherapy, to which she is compliant.
