Mrs. Khan is a 68-year-old female that presents to the emergency department with acute confusion, lethargy and headache. She reports that she feels nauseated but has not vomited. The patient explains that she has felt "off" since she fell off her horse two days ago. She does admit to hitting her head on the ground and sustaining a laceration to the back of her head. She reports that she was alone when she fell and does not recall the fall "I just recall finding myself on the ground". She did not seek medical care following the injury.
Mrs. Khan has a history of atrial fibrillation and asthma. She uses puffers just as needed and is on a daily anticoagulant.
Her vital signs are BP 170/65 HR 52 RR 24 SP02 99 on room air.
