You work for the local health department, and your Director, Dr. Dana Foster, is looking for an opportunity to establish a local partnership that shares resources to meet a public health need in the community.
The Patient Protection and Affordable Care Act of 2010, 26 U.S.C. § 501(r), requires all 501(c)(3) hospitals and medical centers to conduct a comprehensive community health needs assessment (CHNA) every 3 years; to prepare report of findings that is easily understood by the general public; and to be accessible on the company's website. There are a growing number of local health departments that are working with non-profit hospitals and medical centers to complete a comprehensive CHNA in their communities.
As a member of the local health department, you assisted in the completion of one of the local hospital's/ medical center's CHNA, which has been posted on their website for ease of public access. Dr. Foster is eager to move beyond assisting with the development of the CHNA and is now looking to use the information obtained from this completed assessment to build a collaborative partnership with the local hospital/medical center to identify and address a public health problem. The leadership team of the local hospital/medical center is equally eager to enter into a partnership, assuming the right opportunity presents itself. To this end, Dr. Foster has asked you to examine the most recent CHNA to see if there is a suitable opportunity to forge a public-private partnership to address a public health problem. Dr. Foster has also asked that you draft an email for her to send to the local hospital/medical center director.
B. Locate a local community health needs assessment (CHNA) from a non-profit hospital or medical center, in Florida and use this as the starting point for identifying a suitable public health problem and a potential public-private partnership.
C. Examine the resources, personnel, and programs available in Grant City. Health and Human Services. Use only Grant City. Health and Human Services, no other organization located in Grant City for this assignment.
D. Consider the professions from both your selected CHNA and Grant City. Health and Human Services that could be utilized for a successful collaboration. A. Identify a non-profit hospital or medical center in a local community in Florida that has a publicly available CHNA. Use this as a starting point for identifying a suitable public health issue and a potential public-private partnership.
B. Identify a public health problem that can be addressed through a potential partnership between health systems, providing evidence of the public health problem impacting the community and that warrants an intervention.
C. Identify at least three potential professions from both your selected CHNA and Grant City Health and Human Services that could be utilized for a successful collaboration. Include the strengths of interdisciplinary teams in public health and why it is necessary to include multiple professions and various systems in public health.
D. Explain how the potential partnership aligns with and supports the mission (or purpose) statement of each organization. Remember, the vision, mission, and strategic goals of an organization reflect its strategic direction.
E. Describe what this partnership might look like in terms of addressing the identified public health problem. In other words, what would the intervention look like that will address this public health problem?
F. Support your response with at least two scholarly resources, less than 5 years old. In addition, include the source of the community CHNA used in your analysis of collaborative partnerships.
