52-year-old woman is experiencing chronic lumbar pain. Pain began 2 years ago and has gradually increased. She rates her pain at 8/10. She has been using acupuncture for pain management, and it is no longer effective. She uses a heating pad at night, which provides some relief.
She is septic and her WBC count is 13,000, has fever of 101, and is very lethargic. She is a recovering substance abuser who has been sober for 30 years. Because of her history, she has avoided using medication unless absolutely necessary.
She is diabetic with a 4cmx4cm ulcer on her right foot. Daily moist to dry dressing change is ordered.
Imaging studies show degenerative disk disease between L1-L2 and L2-L3. She is prescribed 2grams IV Piggyback Rocephin antibiotic daily. For pain relief she is receiving morphine 2-4mg IV push every 3 hours as need. For her diabetes she is taking oral metformin.
