Evaluation Assignment Instruction.
Evaluation takes many forms for the purposes of developing effective health programs and measuring the value of the programs.
Please respond to the questions below in the form of a Word document. Please upload the completed document to the Blackboard
Grading will be based upon these factors:
• Comprehensiveness of responses to the questions
• The extent to which the responses demonstrate a thorough reading of the textbook and other assigned materials (available on Blackboard)
• The extent to which the selection of indicators demonstrates a multi-level, socio-ecological approach to the program plan
• Readability of responses and quality of writing, including spelling and grammatical considerations
• Compliance with the assignment instructions, including the due date

1. In your own words, please define the various types of evaluation:

2. Please identify the indicators you will be using to evaluate your program plan.
For each indicator, identify the type(s) of evaluation for which the indicator will be used. For example, is it a process indicator, a summative indicator?
In addition, please identify how you will be measuring the indicator - how and when you will gather the information, whether or not you will be using baseline information, how you intend to analyze the information, etc.
Please be sure to include a variety of indicators that demonstrate a multi-level, socio-ecological approach to your program plan.
