For a patient with liver cirrhosis, hypertension, alcohol abuse:

Question: Choose three NANDA approved nursing diagnoses for your patient. Please include one actual diagnosis, one at-risk diagnosis, and one wellness diagnosis. Discuss two patient goals related to each nursing diagnosis (one short-term and one long-term for each diagnosis). Patient goals must be presented in SMART format.

Describe five interventions for each diagnosis, and describe how the interventions would be carried out (I. E: What skills or supplies does the nurse need). These interventions should improve patient well-being, and include teaching (and patient response - if applicable) and nursing care for the patient and family. Interventions should take into consideration the patient's personal health goals and interprofessional health care team collaboration necessary. Please describe the evidence-based rationale for the intervention choice, and provide an appropriate citation.

Evaluate your plan of care with a focus on patient goals. Discuss whether these goals were met, partially met, or unmet; and what your next steps will be. This should include new goals, changed goals, or changed timelines.
