Mr Smith is an elderly client with dementia. His sense of vulnerability has increased in recent years as the dementia has taken hold and he has come to rely upon the support of professional carers and family members, including his wife. His levels of anxiety and stress have also increased. However, his carers have managed to relieve some of the negative impacts through the development of daily routines and Mr Smith's involvement in activities such as shopping and gardening. Mr Smith has also enjoyed the opportunity to engage in discussions with the carers about his life. There are a variety of service activities and steps that need to be taken as means of supporting Mr Smith on an everyday basis. Memory aids must be taken to remind Mr Smith of the location of objects and items that he uses regularly. Mr Smith must also be assisted when dressing and washing. Adaptations will have to be made to Mr Smith's toilet for minimum stress and inconvenience. Medication for pain relief and sleep should also be administered on a daily basis. Evaluation strategies must be implemented in order to gauge the level of success specific to the care and support activities. Regular staff meetings must be arranged and evidence including feedback from Mr Smith and his family members must be taken into consideration. Contingency plans must also be developed in case the provision of care and support proves ineffective or problems are encountered.
