In adult patients diagnosed with type 2 diabetes (DM2), does implementing a diabetes self management education (DSME) program compared to usual practice impact fasting blood glucose (FBS) over 8 weeks? The IRB Prescreening supplement form is used to determine if preliminary activities need full IRB review. It requires information from the DNP readiness form such as identifying the practice project, population, and week to week implementation plan. My data collection plan involves conducting a pre questionnaire to patients to assess their knowledge and conception of their disease process. The questionnaire is available to the public and provided by diabetesinitiative.org as a resource to assist patients and providers. During project implementation fasting blood glucose readings will be collected on a daily basis. By the end of the project, fasting blood glucose levels will be compared to determine if there was an improvement. After the eight weeks, the same questionnaire will be provided to patients to determine if their knowledge education and perception of the disease process has changed. Completion of the prescreening supplement form determines level of risk, type of research and complexity of research design to ensure projects are ethical and compliant with regulations (White, 2020).
