Case Scenario
You are seeing Ms. Clara Johnson, a 32-year-old white female, in the office today with a chief complaint of "I just feel like I can't get my mind to stop racing, and it's making It hard to focus on anything."
• Johnson appears slightly disheveled, but is quiet and polite. She sits frequently fidgeting with her hands and avoids direct eye contact today.
• She reports excessive worry over several aspects of her life and feels this has been steadily worsening over the last year.
• She also reports frequent neck and shoulder pain, fatigue despite sleeping 7 to 8 hours a night, and difficulty concentrating.
• She has a past medical history of mild seasonal allergic rhinitis and takes cetirizine intermittently for this.
• She is married and lives with her husband and one child.
• He does not have any similar pain or swelling elsewhere.
• Upon examination today her BP is 118/76, HR 82, RR 16, and temp is 98.6F. She is alert and oriented, and auscultation reveals a regular heart rate and rhythm and clear lung sounds in all fields. No neurologic deficits on exam. No hallucinations or delusions. Thought process is logical but dominated by worries.
