Question

You are seeing a 26-year-old female for a follow-up visit. You have been working together for the last three years for treatment of bipolar I disorder. Her history is positive for three lifetime psychiatric admissions and two lifetime suicide attempts (one via attempted hanging; one via prescription medication overdose). She has experienced psychotic features during two manic episodes and at least two depressive episodes. Her symptoms have been well managed on a combination of lithium carbonate, Lamictal (lamotrigine), and Abilify (aripiprazole) for the last two years. She and her husband would like to start a family. She explains that her OB/GYNE has recommended coming off of all psychotropic medications before attempting to conceive and would like her to stay off of medications throughout the pregnancy.
