Problem
Background:
Mrs. Jun is a 48 year old woman who has been at Mount King hospital for the last four weeks recovering form pneumonia and sepsis. She was recently transferred to the transitional care unit in hopes of going back to her home in one week. Two nurse Mrs. Jun has bonded with are Lucy and Meg. Lucy has been a nurse for 25 years and has worked all of those years on the transitional care unit. Meg, a nurse for two years, just started working in the unit 3 weeks ago and has been orienting with Lucy since she started. Lucy has decided to have Meg assume the role of Mrs. Jun's care nurse.
Medical History:
Mrs. Jun has a history of hypertension, rheumatoid arthritis (which has limited some of her physical functioning), anxiety, and depression, and newly diagnosed with severe asthma. She developed pneumonia after exposure from a 7th grade girl who she works with who is diagnosed with moderate autism. It was determined she may have been exposed to the pneumonia strain two weeks prior to symptoms. However, she waited ten days after symptoms presented before seeking medical care. Upon admission to the hospital, she had two chest tubes place for empyema, was admitted to the ICU and intubated. She remained intubated for two weeks and sedated during that same time frame due to severe bronchial spasms. She was extubated two weeks ago and has been recovering, but at a slow pace.
Personal and Social History:
Mrs. Jun is widowed with no children. Her husband, a major in the Army, served in Iraq. He committed suicide 10 years ago due to PTSD or post-traumatic stress disorder suffered from atrocities he witnessed in Iraq. She has discussed little about the time after her husband returned from Iraq up to his death. She works as a para (teacher assistant) at the local middle school where she has developed many friendships with the students. Mrs. Jun's only family is one sister who lives in the same town as Mrs. Jun. Mrs. Jun and her sister talk on a daily basis.
