Question

A 75-year-old male client is admitted with a report of pain in the right shoulder. The client has a history of a right hemisphere cardiovascular accident (CVA) 3 years ago that has left him with severe weakness on the left side of his body. He continues to neglect his left side and requires frequent reminders to change position while in his wheelchair. The client requires assistance to transfer from the bed to the chair and was unable to use a walker even before his shoulder pain began. He is incontinent of bladder and bowels approximately 70% of the time. He eats 70 to 85% of all meals but requires frequent prompting to drink fluids.

1. Identify the client findings that pose the greatest risk to this client for the development of a pressure ulcer.
