Susan is a 37-year-old mother of two who has developed an acute anxiety state which is expressed as both a phobia of the dark and also as an obsession centred on hygiene. During your first session you have established that Susan also suffers from bipolar disorder and has been on medication since her early twenties. Susan normally has person-centred counselling but her GP and husband are keen for her to try CBT, which is why she has come to see you. The first session was fairly short as Susan had a panic attack twenty minutes into it, but you gleaned enough information to establish the above problems.

Example dialogue for the second session which demonstrates negotiation of the way forward.

Summary of how you propose to programme the CBT for Susan and include any planned behaviour experiments, activity scheduling etc. and the rationale for their incorporation.
