Question

Kumar and Waris are parents of a four-year-old boy named Halimo. They are a refugee family who immigrated to the area from Somalia two years ago. Halimo has not met some of his developmental milestones, and isn't ready for kindergarten. We will be holding an interprofessional care conference with his parents, the clinic's social worker, and a preschool teacher who was part of the screening process. I'll be facilitating the care conference. The role of the PHN in this care conference is to lead the meeting and help create and communicate the plan for the family. While the PHN will assist in coordinating the follow-up plan, this is not the PHN's primary role at this care conference. The PHN does not conduct a formal speech assessment, but often will screen to see if additional evaluation is needed. The PHN does not serve in a formal therapist or counselor role. Masters-prepared social workers often have licenses that allow them to provide counseling services. It's the role of the social worker to provide suggestions of community resources and financial resources that are available to the family. If a social worker was not available or present, the PHN may need to serve in this role. There are many assessments done during a pre-kindergarten screening to check for deficits, including hearing and vision screenings, fine and gross motor skills, speech and language, and an assessment of cognitive development to determine whether the child is meeting developmental milestones. Although an assessment of dietary intake and nutritional status is important, it is not usually included in pre-kindergarten screening assessments. Likewise, a screening for communicable diseases, though important, is not usually included in pre-kindergarten screening assessments.

Using a family perspective: Identify 2 client/family needs; develop a therapeutic nursing statement/response from the nurse directed toward the client/family that addresses the identified client/family needs; and develop 2 nursing interventions that address the identified client/family needs.
