The NDIS Review (2023) and the Improving Outcomes for All Report (2023) both point to the need for greater collaboration among the education, allied health health and community sectors if better outcomes are to be achieved for those living with disability or disadvantage. a template to be used when writing individualised plans. This need not be original (there are many templates on the internet) but your reasons for choosing it must be given and supported. Make sure you acknowledge the source of the template. You should fill in the template as an exemplar for a child/student/family (real or imagined) so that there is a concrete example of how the template is to be used (approx. 600 words)
