EVALUATION PROPOSAL
Develop an evaluation proposal, suitable for submission to a funding organization. You will select an existing program to evaluate or design.
Details on your evaluation proposal are introduced at the Module 1: Checkpoint and in Module 7, with this assignment. A number of outlines that you could select from to guide your proposal development have been provided. You can tailor the outline you choose to fit your research needs, depending upon the type of evaluation you select. You can either create your own program design or, alternatively, you can find an existing program to develop an evaluation proposal for. For this course, you do not need to complete the evaluation. There is not sufficient time in the course to do that. You only need to develop an evaluation proposal that a prospective funder would select. The paper should be 15 pages in length, typed, double spaced, one-inch margins, 12-point Times New Roman, and be in APA format. Include a budget, instruments, such as surveys, a resume for the evaluator, and a logic model. Provide any necessary information that supports funding your proposal. Please contact your instructor if you have questions about this assignment.

Preparing for an Evaluation
What program would you like to evaluate?
Why do you want to do an evaluation?
What it is that you want to le arn about your program?
Who are your decision-makers? (Will you have the ir support if you de cide to evaluate your program? What will you do if you don't have the ir support?)
What sort of time, resources, and staff do you have available to he lp you conduct an evaluation?
Write one or more ways that you might get your stakeholders involved and inve sted in your evaluation.
Overview of Program to Be Evaluated
What is the overall goal of your e nvironme ntal public he alth program (or the program you would like to e valuate )?
What are your program's objectives?
Overview of Program to Be Evaluated (continued)
What are your program's activitie s? List them and briefly note why you do that activity (how does it he lp you reach the goal?).
Logic Model
This logic model template can he lp you organize information about your program activities and the results. Refer to the module for explanations of e ach category.
Analyze the Information
Think about how you will analyze the findings and draw conclusions from the data you have collected. Consider the following que stions.
What are your criteria for success in your program?
To what will you compare the information you collected?
What information might still be needed in order to justify your conclusions (for example, statistical information, financial information)?
Analyze the Information
This table may be useful for thinking about how to analyze diffe re nt type s of information you have collected.
An example of an outline for an evaluation plan for an asthma program, which could be modified into an evaluation proposal to a funder, is provided (Centers for Disease Control and Prevention, 2014 (Links to an external site.)). You can use this model to develop your own evaluation proposal to a funder. Additional resources for an evaluation proposal are available from files provided by (Harris, 2010) contained Your Life, Your Health!
You may use any of the suggested resources as a model to develop your own evaluation proposal to a funder.
