Over the past two weeks of my field experience, I have become very familiar with the resources and systems that social workers use to connect patients with services that support a more person-centered discharge. This has helped me become more efficient at helping patients transition and maintaining continuity of service. This experience has also boosted my confidence as I move into the field, as it has improved my communication and documentation skills. One thing I plan to change in the upcoming week is to be more proactive and less reactive. I plan to take a deeper dive into the client's chart to foresee the services and resources the patient might need upon discharge. I plan on using supervision in the upcoming week to help me understand how a deeper dive into the client records prior to discharge can be beneficial to the client and me.
